Human Failure causes accidents but not Human Factors. On the contrary I regard our Human Factors as a fantastic construction or design, which contributes to making our lives good and gives us a positive time on earth. In my opinion, it is when our Human Factors stop functioning, that accidents occur. If every individual is "in the loop", the risk for human failure will be reduced."
