Form 6. Declaration Of A Desire for a Natural Death - does serious act of stopping care if later doctors think incapacitated person's health is very bad and more care won't help. (this is often called a "Living Will").
Form 7. Do Not Resuscitate Order - does serious act of saying immediately from now on do not try certain health care, and this actually is 2 forms either a "D.N.R." form about "resuscitation" and not trying to help breathing or heart, or a "P.O.S.T." form about not trying many medical treatments listed in the form.

Form 8. Durable Power Of Attorney -
lets power over money, property, and more be given during life to trusted person so they have legal power to help do things, like handle paying bills, and this book's form also has option to give power over a minor child under 18 but South Carolina law is not clear about this.
Form 9. Final Wishes - lets instructions be given and maybe person named to control issues with body after death like funeral or burial (usually done if person doesn't want closest family to do this like usual).
